Certification Package

This document sets forth the certification program overall. The Application Fee Schedule is included as Appendix C.

Certification Questionnaire

This document is the questionnaire that applicants for LIHI certification must fill out; it provides the criteria, and standards, and indicates whether a Facility will "pass" or "fail" depending on the responses. Potential applicants are encouraged to review this questionnaire and refer back to the criteria and certification package for additional information and instructions.
